it in any of the following ways:
- Either fax the form to (852) 2788-8561;
- Or scan the completed form and send it
to us by email;
- Or fill the form with MS Word (or any
compatible program, e.g. OpenOffice or Mac's TextEdit), produce a pdf,
and send it by email to focm2008@cityu.edu.hk. Note that in this
case (if you are paying by credit card) you do not
need to sign the form.
Please fill the form with your details even if you
defer the payment of the registration fee until your arrival.
